Canadian Health Care navigation program which provides answers, guidance and support to its customers

The healthcare navigation platform services provide comprehensive and personalized support to members and their families during illness or other health crises.
The project aims to turn the program into a digital program with an opportunity to get better data, more engagement with the patient, upload medical records/notes online, and better information sharing between the customer, clinician, and nurse.
Nurse Case Managers provide personalized support and expert navigation of the healthcare system to improve the overall quality of care, recovery, and outcomes.
It's designed to promote prevention and early intervention with the help of educational materials geared towards self-management, mobile therapy, and modules that measure progress during and between therapy sessions.
Once we understood the problem and gathered all relevant information, we brainstormed ideas and potential solutions in the next step through sketching, collaborative workshops, and other ideation techniques.
Our potential solutions led us to define the style and create prototypes and mockups of the ideas.
Once the design had been refined and tested, the final step was to implement the solution. It involves working with developers and engineers and testing the platform.
One of the most challenging discoveries we encountered throughout our ideation sessions and testing possible solutions was the potential problem of overwhelming users with medical terms and a lengthy assessment process.
As a solution, through research and the help of a medical professional, we've formed a user-friendly vocabulary and separated the assessment process into smaller sections tailored to specific programs and user needs.
It is a well-rounded platform that provides four programs for different health and personal states and needs that users can enroll in.
Each program is created explicitly for specific users' needs and health situations where users can assess their health and well-being.
Communicate with the care professionals through messages or audio/video calls about their needs and states and receive guidance and support when needed.
